✓ I fully understand that I am solely responsible for the truthfulness and accuracy of all the information provided to Club B&M.

✓ I hereby authorize Club B&M to complete and submit the TWAC (Taiwan Arrival Card) on my behalf, using the information I have provided. I understand that Club B&M will act in good faith based on the information provided by me.

✓ I understand that it is my responsibility to ensure that all information provided is accurate and up-to-date. I will promptly inform Club B&M of any changes to the information provided that may affect the TWAC (Taiwan Arrival Card) and in accordance with the instructions of the National Immigration Agency of the Ministry of the Interior.

✓ I understand that if the information I provide is incomplete, unclear, inaccurate, fraudulent, or conceals facts, it may have consequences as outlined in the relevant regulations or the document itself, for which I will be solely responsible and I may be subject to a prohibition on entry, and I will bear legal responsibility for the unlawful act.

✓ If I fill out and submit TWAC information on behalf of my accompanying traveler, and the information is incomplete, unclear, inaccurate, fraudulent, or conceals facts, the traveler may also be subject to a prohibition on entry. Both the traveler and I will bear legal responsibility for the unlawful act.

✓ I have read, fully understood, and agree to this declaration, and I give Club B&M the authority to act on my behalf for the sole purpose of completing and submitting the TWAC (Taiwan Arrival Card).

✓ If I am filling out and submitting the TWAC on behalf of my accompanying travelers, I affirm that the traveler has clearly acknowledged the information I have filled out and submitted, and I am authorized to submit this information as if it were submitted by the traveler personally.
